Request a Sacrament Certificate

Do you need a certificate of your Baptism, First Communion, Confirmation, or Matrimony?

Information needed on the email is as follows:

1. Name (as it appears on the birth certificate)

2. Date of Birth

3. Date/Year of the Sacrament (if known)

4. Which church: St. Symphorosa or St. Rene Goupil

5. Names of both parents - including mother's maiden name (*for baptism)

*If you are requesting a Baptismal certificate,

please provide a copy of a driver's license or state ID.
