Request a Sacrament Certificate

Do you need a certificate of your Baptism, First Communion, Confirmation, or Matrimony?

 

Please email sacraments@twoholymartyrs.org to get your request started.

Allow up to 7 business days for your request to be completed.

  Information needed on the email is as follows:

1. Name (as it appears on the birth certificate)

2. Date of Birth

3. Phone number - in case we have any questions about your request.

4. Which Sacrament,  Date/Year of the Sacrament (if known)

5. Which church: St. Symphorosa or St. Rene Goupil

6. Names of both parents - including mother's maiden name (*for baptism)

 Once the certificate(s) are completed someone from our office will contact you.

 

 

*If you are requesting a Baptismal certificate,

Please provide a copy of your driver's license or state ID with the email.
